As reported for this event by the customer, the load lcg function could not be executed for the device.The disinfectant preparation for reprocessing could not be completed.The drain disinfectant process had completed successfully.There is no patient involvement.

|
Field service engineer (fse) went on site to troubleshoot the customer¿s issue of being unable to load the disinfectant.Fse investigated the device to ensure that the device was functional.Fse worked with the customer through several steps of working the device.The device was functional and no repairs were needed.It appeared that the customer had inadvertently mixed up some steps which may have led to the issue.Evaluation is ongoing.Supplemental report(s) will be submitted when any relevant new information is available.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, it is likely the switching valve was not completely closed.Therefore, disinfectant that was once pumped up to reprocessing basin for discharge returned to disinfectant tank again, which led to occurrence of the suggested event.Olympus will continue to monitor field performance for this device.
