The customer reported that the distal end of her olympus videoscope tore off and had no eto cap.Additional details relating to the patient and the event have been requested, but no response has been received at this time.There was no patient harm or consequence reported as a result of this event.

The device was returned, and an initial evaluation was conducted by olympus; however, investigation is ongoing.During the initial evaluation, the user report was confirmed, the distal end of the unit had torn away.Consequently, the device failed the scope leak test due to a leakage at the distal end.If additional information becomes available following the device evaluation, a supplemental report will be filed.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 11 years since the subject device was manufactured.Based on the results of the investigation, the cause of the distal end of the unit being torn away likely occurred from stress and/or user handling the device.The specific root cause could not be determined at this time.The following information is stated in the instructions for use: "chapter 3 preparation and inspection.3.2 inspection of the endoscope: 3.Inspect the external surface of the entire insertion tube for dents, bulges, swelling, peeling or other irregularities.4.Holding the insertion tube gently with one hand, carefully run your fingertips over the entire length of the insertion tube in both directions.Inspect for any protruding objects or other irregularities.Also confirm that the insertion tube is not abnormally rigid." olympus will continue to monitor field performance for this device.
