Event Description
It was reported that approximately five years three months post port placement via right subclavian vein, the catheter allegedly had leakage and was found to be broken.Reportedly the catheter migrated inside the patient.It was further reported that the distal catheter segment and the port body were removed from patient.The patient also experienced discomfort and pain.The patient current status is unknown.
 
Manufacturer Narrative
Manufacturing review: a manufacturing review was not requested as the lot number reported is unknown.Investigation summary: one powerport isp mri attached to a groshong catheter in two segments was returned for evaluation.Functional, gross visual and microscopic visual evaluations were performed.The investigation is confirmed for the reported catheter fracture, leak, material separation and the identified wear and deformation issues, as a complete compound break was noted approximately 4.3cm from the distal end of the cath-lock.A complete circumferential break was noted to the proximal end of the distal catheter segment.The edges of the complete compound break on the proximal segment were noted to be sharp in one region and rounded in another.The surface of the complete compound break was observed to be rounded in one region and jagged in another but glossy throughout.The distal complete compound break edge was noted to be smooth and sharp; the surface appeared granular in one region and rounded in another.The investigation is inconclusive for the reported migration issue, as the exact circumstances at the time of the reported event are unknown, and the reported event could not be reproduced in the lab.A definitive root cause could not be determined based upon the available information.Labeling review: a review of product labeling documentation (e.G., procedural instructions, indications, warnings, precautions, cautions, possible complications, contraindications, nursing guide, and unit label) did not find any product labeling inadequacy.
