Olympus technical assistance center (tac) support called customer to troubleshoot the device.Customer reported that there was no connection from the pc to the cv-180 device.Tac advised customer to reset the cv-180 firmware and select cvp under user settings, and the issue was resolved.The investigation is ongoing; therefore, the root cause of the reported event cannot be determined at this time.However, if additional information becomes available this report will be supplemented accordingly.

A user facility returned the olympus evis exera ii video system center due to the device not connecting to the personal computer (pc) and not displaying any image.There was no harm or user injury reported due to the event.

|
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.The subject device was manufactured on a date that was not identified.The device history record was unable to be reviewed for this device.However, olympus only releases products to market that meet all manufacturing specifications and final product release criteria.The case is not due to design.Based on the results of the legal manufacturer's investigation, it was presumed that the indicated phenomenon was caused by a firmware defect, but the cause of the firmware defect could not be identified.There was a problem with the cvp selected in the user settings that caused the error to the firmware resetting.The issue was resolved.Olympus will continue to monitor field performance for this device.
