|
It was reported to olympus that during preparation for use, the evis exera ii video system center image kept turning to different colors (green and pink) and the device processor keys did not work.There was no patient harm or user injury reported due to the event.

The device was returned for evaluation, the evaluation was unable to confirm the reported event.The device was assessed with 150,180 and otv-s7pro camera head and all these devices worked with the processor.Additionally, the processor was kept for 7 working days and no issues were found.The investigation assumed the reported event might be due to power fluctuation at the customer site.The investigation is ongoing; therefore, the root cause of the reported event cannot be determined at this time.However, if additional information becomes available this report will be supplemented accordingly.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 10 years since the subject device was manufactured.Based on the results of the investigation, and the inability to confirm the issue, it is unknown whether the phenomenon occurred due to the malfunction of the device in question.A definitive root cause cannot be identified.Olympus will continue to monitor the field performance of this device.
