The device was inspected by a field service engineer and the main lamp failed to light up however, the spare lamp came on.The malfunction was caused by a faulty converter.The device will not be returned for further evaluation and repair.The investigation is ongoing; therefore, the root cause of the reported event cannot be determined at this time, however, if additional information becomes available, this report will be supplemented accordingly.
 
Event Description
The customer reported the visera elite xenon light source lamp is not working.It was reported that the event occurred during reprocessing , prior to a diagnostic procedure.A similar device was used to complete the procedure with no more than a 15 minute delay.There was no patient harm or user injury reported dude to the event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the legal manufacturer's investigation, it was presumed that the lamp ceased to light due to a failure of the converter.The cause of the failure of the converter could not be estimated.Olympus will continue to monitor field performance for this device.
