Event Description
The customer reported and opened a repair request for the visera cysto-nephro videoscope because the terminal sheath was broken.The issue was discovered after the procedure.No patient harm or injury was reported.
 
Manufacturer Narrative
The suspect device has been returned to olympus for evaluation and the investigation is in process.The olympus service center reported that the bending rubber was missing and stated that the defect was caused by customers handling or wear and tear.In addition, the repair center found the following defects: the coating on the connection tube was peeling, wrinkled and discolored; the scope body coating was peeling off; the universal cord, cable tube unit coating and the connector plug unit were damaged.A definitive root cause of the reported event cannot be determined at this time.If additional information becomes available, this report will be supplemented accordingly.

This report is being supplemented to provide additional information, based on the legal manufacturer's final investigation.A review of the device history record found no deviations, that could have caused or contributed to the reported issue.It has been over 13 years, since the subject device was manufactured.Based on the results of the investigation, it is likely that this phenomenon occurred, due to the same cause as other breakages.It was not possible to determine, whether the breakage was, due to stress, handling, or other factors.A definitive root cause cannot be identified.Olympus will continue to monitor the field performance of this device.
