A field service engineer (fse) was onsite to troubleshoot the issue and the reported e101 error was confirmed.The fse performed general operation and safety checks with positive results.In addition, the fse found dust that blocked the air flow of the fan and the dust was eliminated with compressed air.Lastly, the xenon lamp maj-1817 was replaced.The device will not be returned for further evaluation and repair.The investigation is ongoing; therefore, the root cause of the reported event cannot be determined at this time, however, if additional information becomes available, this report will be supplemented accordingly.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the legal manufacturer's investigation, it is likely that this phenomenon occurred because the fan supply was cut off due to a large amount of dust inside the airframe.The instruction manual identifies the following related verbiage that may help prevent the phenomenon: ¿avoid using the light source in a dusty environment.This may damage the light source.Clean and vacuum dust the ventilation grills using a vacuum cleaner.Otherwise, the light source may break down and gets damaged from overheating.¿ olympus will continue to monitor field performance for this device.
