Event Description
A user facility reported to olympus that water was leaking into the olympus, model clv-180, evis exera ii xenon light source and the picture was distorted; the color was observed "bluer than normal." this report is submitted due to a report that water was leaking into the device.There was no patient injury, associated with the problem, reported to olympus.
 
Manufacturer Narrative
As the device was not returned to olympus for evaluation, a root cause for the reported event cannot be determined.If additional information becomes available or the device is returned for evaluation, a supplemental report will be submitted.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 14 years since the subject device was manufactured.The device was requested to be returned for evaluation; however, after three good faith attempts to reach the customer, the unit has not been obtained.Based on the results of the investigation, a definitive root cause could not be established.It was not possible to make a further determination on the subject device.Olympus will continue to monitor field performance for this device.
