Event Description
It has been reported that the bd vacutainer® ultratouch¿ push button blood collection set has been found experiencing retraction issues".The following has been provided by the initial reporter: according to the customer's verbatim report, the needle was not in the specified position, and the hcp pressed the button but the needle was not retracted.

Date of event is unknown.The date received by manufacturer has been used for this field.A device evaluation and/or device history review is anticipated, but is not complete.Upon completion, a supplemental report will be filed.

The following fields were updated due to additional information: d10: device available for eval yes.D10: returned to manufacturer on: 2022-01-20.H6: investigation summary: bd received 1 sample and 1 photo for investigation.The photo was reviewed and the indicated failure mode for retraction failure with the incident lot was not observed.Additionally, the customer sample along with 30 retention samples from bd inventory, were evaluated by visual examination and functional testing and no issues were observed relating to retraction as all samples met specifications.Based on a review of the device history record for the incident lot, all product specifications and requirements for lot release were met.There were no related quality issues during manufacturing of the product.This complaint is unable to be confirmed for the indicated failure mode retraction.Bd was not able to identify a root cause for the indicated failure mode.
