The suspect device has been returned to olympus for evaluation and the investigation is on-going.The olympus service center confirmed there was oil leakage inside of the product due to a defective regulator.The following defective parts are required to be exchanged: manifold, proportional valve, regulator unit, s-joint.However, the alarm sounds were functioning normally.If additional information becomes available, this report will be supplemented accordingly.
 
Event Description
The customer reported to olympus that the high flow insufflation unit alarm sound was not clear and requested to check whether the alarm works normally when the pressure drops.The reported problem was found during an unspecified procedure.No patient involvement or injury was reported.Upon inspection of the customer returned device, oil leakage inside the product was found.This report is being submitted for the event found at evaluation.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, a definitive root cause could not be established.From the results of inspection, it was confirmed that an oil leak was found inside of the product.This was due to a broken regulator.However, there was no assumed cause associated with the failure of this component.Olympus will continue to monitor field performance for this device.
