Event Description
It was reported that a minicap transfer set had connection issue; further described as "unable to connect to the titanium adapter due to the patient connector is malformed".This was observed before use of the device for peritoneal dialysis therapy.There was no patient involvement.No additional information is available.
 
Manufacturer Narrative
H10: one actual sample was received for evaluation.A visual inspection with the naked eye noted the patient adapter was malformed on the outer portion and contained flash on the inside threads of the adapter.Functional leak testing was performed with an in lab titanium adapter and a leak was observed at the patient adapter and titanium adapter connection.The patient adapter would not fully seat onto the titanium adapter.Clear passage testing was performed with no issues noted.The reported condition was verified.The cause of the condition was due to the injection molding process during manufacturing.A batch review was conducted and there were no deviations found related to this reported condition during the manufacture of this lot.Should additional relevant information become available, a supplemental report will be submitted.
