The suspect device has been returned to olympus for evaluation and the investigation is in process.The olympus service center found in addition to b5, unit automatically powers on when plugged in due to the damage to the power switch.This unit does have an outdated power switch and will require upgrading.Cosmetic damage found.Front panel is cracking around the scope socket area and the lamp button has a hole in it.Lamp has 500 or more hours used.The faulty parts were replaced.The device was inspected and passed olympus functional standards.A definitive root cause of the reported event cannot be determined at this time.If additional information becomes available, this report will be supplemented accordingly.
 
Event Description
The user facility reported to olympus the evis exera iii xenon light source had a stuck power button.Upon inspection and testing of the returned device, it was observed that the device had a power switch failure.This report is being submitted for the event found during estimation.There was no harm or user injury reported due to the event.
 
Manufacturer Narrative
This supplemental report is to inform that upon further review, this is not a reportable malfunction.Per the legal manufacturer, there is no potential for this issue to cause or contribute to death or serious injury if the malfunction were to recur.
