During troubleshoot via telephone, detergent was confirmed in the detergent tank.The detergent bottle connector was connected appropriately and there were no issues with the tubing.The support engineer informed the customer that once the detergent sensor senses detergent flow the detergent lamp will reset.The biomedical engineer will advise the operator of the device to attempt a cycle and proceed.The investigation is ongoing; therefore, the root cause of the reported issue cannot be determined at this time.However, if additional information becomes available a follow up medical device report will be supplemented accordingly.

Olympus technical support engineer was informed by the biomedical engineer at the user facility via telephone that during reprocessing, the automated endoscope reprocessor (aer) machine¿s ¿detergent lamp illuminated, no error codes present¿.No death, injury or harm was reported to olympus.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation and to correct d9.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, it is likely the detergent replacement indicator did not disappear since reprocessing was not performed after replacing detergent bottle.Olympus will continue to monitor field performance for this device.
