During troubleshooting the customer was advised to verify all gauges around fifty (50) for all of them, select the rinse function the customer informed the service center representative all oer-pro gauges dropped to thirty (30).After the rinse cycle completed the customer confirmed the gauges is now at twenty-nine (29).Lastly the customer was instructed to purge the air from the internal water filter, by connecting tubing and selecting leak test to verify the time code is holding at twenty-nine (29).The investigation is ongoing, and a definitive root cause of the reported event cannot be determined at this time.If additional information becomes available, this report will be supplemented accordingly.

It was reported to olympus that the after replacing the external water filters and the internal water filter on the oer-pro endoscope reprocessor, the time code on the cycle is thirty-one (31).There was no reports of patient death, harm, or injuries related to this event.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, it is likely the water supply in the reprocessing basin was weaker than before because of inadequate air drainage from the water filter.To prevent recurrence, the instructions for use advise the user to ensure air drainage when replacing water filter.Also, low water supply from the main valve at the facility was another likely caused.Olympus will continue to monitor field performance for this device.
