It was reported to boston scientific corporation that a trapezoid rx basket was used in the common bile duct during an endoscopic retrograde cholangiopancreatography procedure performed on (b)(6) 2022.During the procedure, an alliance handle was used in conjunction with the trapezoid basket in an attempt to crush a large stone (no specific cm size given), but was unsuccessful.Additionally, the tip failed to detach from the basket to release the stone.The basket was then cut below the handle of the catheter and a lithocrush device was used by inserting the catheter wire into the device and cranking it, but this was also unsuccessful.A 12-15mm cre balloon was then used to perform sphincteroplasty, which ended up disengaging the stone from the basket.There were no patient complications as a result of this event.
