The device was returned to olympus for evaluation and the customer¿s allegation was not confirmed.The device evaluation found corrosion on the top cover, bottom chassis, rear panel and video connector pins.In addition, the software was required to be updated as it was out of date.The investigation is ongoing and follow up with the user facility is currently being performed.Therefore, the root cause of the reported event cannot be determined at this time.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
A customer reported to olympus, there was an issue with the power button evis exera ii video system center prior to an unknown procedure.There were no reports of patient harm associated with this event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's investigation.The dhr was unable to be reviewed for this device because it was manufactured more than 15 years ago.However, olympus only releases products to market that meet all manufacturing specifications and final product release criteria.The case is not due to design.Based on the results of the legal manufacturer's investigation, the root cause of the reported event could not be identified, nor could the phenomenon be confirmed/reproduced.Olympus will continue to monitor field performance for this device.
