This event was discovered while inspecting a returned loaner device.During this device inspection, it was also noted that the lamp life meter was reading at below 50 hours with an output measuring within range.The scope socket, igniter cable, fan and air pressure were all up to date and functioning properly.If additional information becomes available following the device evaluation, a supplemental report will be filed.
 
Event Description
During receipt inspection of a returned loaner evis exera ii xenon light source, a small screw was noted missing on the back of the main printed circuit board.This event had no patient contact.
 
Manufacturer Narrative
Correction to g3 of the initial medwatch.The aware date should be 11-feb-2022.Investigation activities have been opened to manage the actions related to this report and any required mdr reporting.This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.Please see the updates in sections h4, h6, and h10.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 13 years since the subject device was manufactured.Based on the results of the investigation, the investigation confirmed the main board back side was missing a screw.The specific root cause of the missing screw on the main board back side could not be determined at this time.The lens base was cracked and the rear foot deformed; however, these defects are not considered severe enough to cause a potential adverse event.Olympus will continue to monitor field performance for this device.
