It was reported that after replacing the "on-off valve", the disinfectant cannot be recycled normally; and connecting the computer to test that the "on-off valve" is abnormal no procedure involved.The fault occurs during reprocessing.There was no patient harm , no user injury reported.

Initial reporter address- full name of establishment is (b)(6).Full address is (b)(6).The subject device was evaluated by (fse)field service engineer onsite.Device evaluation , fse noted the switching valve was faulty, the computer connected to detect shows that the on-off valve cannot work normally.Investigation is ongoing.This report will be supplemented accordingly following investigation.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the following likely caused the event.Disinfectant was drained to the water drain side due to a malfunction of the switching valve when the disinfectant was supplied from the reprocessing basin.The root cause of why the switch failed could not be determined.Olympus will continue to monitor field performance for this device.
