Event Description
|
Medtronic received information regarding an imaging system being used during a spinal procedure.It was reported that a beeping sound was intermittently coming from inside the system causing it not to expose.The system had to be restarted to enable it to take 3d images.Logs were reviewed by a manufacturer representative (rep) and found a "recoverable error 134." the voltage supply for the microcontroller u21 was out of spec.There was no reported impact on patient outcome.

Event Description

Additional information received from a manufacturer representative indicated the site was able to perform an initial image acquisition earlier in the procedure, but they were unable to perform an image acquisition midway through the procedure due to the reported issue.A c-arm was used to confirm screw position.The issue resulted in a procedure delay of approximately 15-20 minutes trying to get to imaging system working after reboots.
