Event Description
It was reported that patient (pt) reports unable to recharge her left ins since (b)(6) 2021 or (b)(6) 2022.It is unknown if there were any contributing factors. pt attempted to recharge and recharger unable to recharge ins.Manufacturer representative (rep) reached out to clinic to request office appt with pt.The issue has not been resolved.
 
Event Description
Additional information was received from the manufacturer's representative who reported that the patient¿s left sided device was in physician overdischarge mode as the patient stated that it may not have been recharged since february 2022.Patient technical services was contacted, the device status was reported and they assisted the rep with getting the device out of physician over discharge mode.The device was brought out of the over discharge mode, recharged to 25% and turned on per the hcp's instructions.The patient was instructed to charge the device to 100% every day for the next two weeks at the very least.The nurse recommended to the patient that the device should be charged to 100% every day on a forward going basis to be on the safer side.
