Event Description
It was reported that a physician was attempting to use a protégé everflex self-expanding stent device to treat a mid-iliac artery ¿ common lesion in in a patient.No damage noted to packaging, no issues noted when removing the device from the hoop/tray.Device was prepped as per ifu.It was reported that premature deployment occurred in vivo, when the stent was positioned, the delivered stent and sheath were ejected at the same time, and it was found that the stent had been deployed and the safety valve had not been opened.The procedure was completed by replacing the product, and the operation was successfully completed.No patient injury reported for this issue.

Manufacturer Narrative
