Event Description
It was reported that there was a connection issue between the female connector of a peritoneal dialysis (pd) transfer set and the patient line of an amia cassette.The caregiver could not disconnect the transfer set from the patient line of the cassette during the end of pd therapy.The patient was connected at the time of the event.To resolve this issue, the transfer set was replaced.There was no report of patient injury or medical intervention associated with this event.No additional information is available.
