Event Description
|
It was reported to boston scientific corporation that an obtryx ii system - halo device was used during a transobturator sling placement procedure performed on (b)(6) 2022 for the treatment of urinary incontinence.During the procedure, the physician had difficulty in tensioning the sling and the mesh curled.Photos of the device taken during the procedure showed the mesh was stretched and curled.The procedure was then cancelled and rescheduled on (b)(6) 2022.When the patient returned, a piece of plastic sleeve from the mesh assembly was found in the patient and was retrieved by hand.The procedure was completed with another obtryx ii system - halo.There were no patient complications reported as a result of this event.
