Event Description
It was reported that an unknown patient underwent an unknown ablation procedure with a carto® 3 system.There was a burning smell in the power cord region.Burning smell in the system cart power cord region.No patient consequences were reported.Fire hazard is mdr-reportable.
 
Manufacturer Narrative
On 8-aug-2022, bwi received additional information regarding the event.The issue was not solved.The medical team is waiting for the appraisal of the equipment division.The root may be malfunction of the electrical converter.They have the report yet regarding if root cause was related to carto; it¿s the responsibility of the equipment division.No errors or messages observed.They did not see any smoke, just the smell.Actins taken were to turn off the equipment.If additional information is received regarding this event, a supplemental 3500a report will be submitted to the fda.Manufacturer's reference number: (b)(4).

On 15-sep-2022, the product investigation was completed.It was reported that an unknown patient underwent an unknown ablation procedure with a carto® 3 system.There was a burning smell in the power cord region.Device evaluation details: field service engineer (fse) arrived at the hospital and checked the system.The fse found that cart system did not present any defects.There was no smell of burning anywhere.The fse performed tests with clinical engineering and head nursing.The system is operational.A manufacturing record evaluation was performed for the carto 3 system # 14549, and no internal actions related to the reported complaint condition were identified.If additional information is received regarding this event, a supplemental 3500a report will be submitted to the fda.Manufacturer's reference number: (b)(4).
