Event Description
|
The customer reported the pin at the back was completely melted.The customer later clarified that the issue was a melted/broken fan, and it was discovered outside of use during the daily check on the ventilator.There was no patient involvement.Further information has been requested.If additional information becomes available at a later date, a supplemental report will be submitted.

|
Field service engineer (fse) was dispatched to customer site.During evaluation fse determined the cooling fan housing was cracked and pins could not hold fan in correct place.Fse replaced cooling fan, fan guard, filter, and retainer.Fse did not find any errors in the diagnostic report (drpta)/event log.Fse performed a performance verification test according to the v60 service manual rev p.Unit passed all test and is ready for use.
