Event Description
The customer reported that, during inspection prior to use, the video connector on the subject device could not be properly connected to the videoscope.The choledochoscopic surgery was completed with another similar device.There was no effect on the patient due to the event.
 
Manufacturer Narrative
Establishment name: (b)(6) hospital.The suspect device was sent to an olympus service center for evaluation.Inspection and testing found the video connector was not damaged to the point where it could not be connected.However, the image was noisy due to a defective connector.The investigation is ongoing; therefore, the root cause of the reported event cannot be determined at this time.However, if additional information becomes available this report will be supplemented accordingly.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the investigation results, it was confirmed that the cause of the event was a faulty 180 connector u (video connector socket).It is likely that it became defective due to long-term use or user handling by applying excessive stress to the connector.The instruction manual of the cv-180 describes the following, and the reported event may have been prevented by following the instructions for use: "do not apply excessive force to this video system center and/or other instruments connected.Otherwise, damage and/or malfunction can occur.Never apply excessive force to connectors.This could damage the connectors." olympus will continue to monitor field performance for this device.
