Event Description
A nurse reported that following implantation of an intraocular lens (iol) a foreign material found on the back of implant, surgeon unsure if it was from the cartridge or something else.Lens was loaded and foreign material appeared on back of the lens, surgeon removed this with irrigation/aspiration.Additional information was requested and received stating the foreign material that is actually still lodged in the patients eye and did not affect the patients vision.Additional information was received stating that the patient is doing well, nothing to report, healing as per normal.

A sample device was not returned for analysis.Complaint history and product history records were reviewed and documentation indicated the product met release criteria.Root cause has not been identified.There have been no other complaints reported in the lot number.The manufacturer internal reference number is: (b)(4).

Event Description
Additional information was requested and received vision is good but she still has a greenish foreign body behind the lens.The conjunctiva was a bit inflamed as she stopped using her prednefrine prematurely.
