Event Description
It was reported that a minicap had a connection issue; further described as ¿when trying to put a minicap on, the minicap would not go on the transfer set¿.This was observed during use of the device for peritoneal dialysis (pd) therapy.It was further reported that ¿the thread on the inside looks totally different¿.There was no report of patient injury or medical intervention associated with this event.No additional information is available.
 
Manufacturer Narrative
H4: the lot was manufactured april 08 2022 to april 14 2022.H10:the actual device was received for evaluation.A visual inspection was performed and damage was noted; approximately half of the thread was not properly formed.The wall of the sample showed signs of deformation/damage along the circumference.Functional testing was performed with an in-lab transfer set and the cap could not be secured to the set.The reported condition was verified.The cause of the condition was related to the molding process during manufacturing.A batch review was conducted and there were no deviations found related to this reported condition during the manufacture of this lot.Should additional relevant information become available, a supplemental report will be submitted.
