It was reported that on (b)(6) 2022 a patient presented with isolated tricuspid regurgitation (tr) and a pressure gradient of 2 mmhg.An off-label procedure was being performed in the tricuspid valve.After the first mitraclip (cds-20222r348) was placed the gradient elevated to 4 mmhg.When the second mitraclip (cds-20224r407) was placed, the gradient further increased to 5 mmhg.Despite the increased gradient, the physician was satisfied with the results.There was no adverse effect due to the pressure gradient.No additional information was provided.

The device was not returned for analysis.A review of the lot history record revealed no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no similar complaints reported from this lot.Based on the information reviewed, the reported off-label use was due to the user performing the mitraclip procedure on the tricuspid valve.A cause for the reported tricuspid stenosis cannot be determined.The serious injury/ illness/ impairment was the result of case-specific circumstances as no treatment.There is no indication of a product issue with respect to manufacture, design, or labeling.
