Event Description
It was reported that a minicap extended life pd transfer had a connection issue; further described as ¿unable to fit tightly with the minicap¿.This occurred after peritoneal dialysis (pd) therapy.The transfer set was replaced.There was no report of patient injury or medical intervention associated with this event.No additional information is available.
 
Manufacturer Narrative
B5: during follow up, it was clarified that the connection issue resulted in iodine leakage between the transfer set and mini cap.H10: the device was received for evaluation.A visual inspection with the naked eye noted a damage in multiple locations of the threads and sealing surface of the dark blue connector.Functional testing including clear passage and clamp function were performed and there were no issues observed.Leak testing was performed with an in lab mini cap connected to the female connector and a leak was observed between the mini cap and female connector.The reported condition was verified.The cause of the leak was due to the damaged female connector.The cause of the damaged female connector was due to a manufacturing related issue.A batch review was conducted and there were no deviations found related to this reported condition during the manufacture of this lot.Should additional relevant information become available, a supplemental report will be submitted.
