|
The customer reported to olympus, the endoscope reprocessor's disinfectant solution counter was not resetting.There were no reports of patient harm.

The device was not returned to olympus for evaluation.The customer was instructed to verify date/time, as well as verifying that the cycle count is not set too low and days is set to 5 as the customer is using acecide.The customer resolved the issue by setting the proper cycle count and days the investigation is ongoing and follow up with the user facility is currently being performed.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.

This report is being supplemented to provide additional information based on the approved final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, and since the device was not returned for evaluation, the definitive root cause of the disinfectant solution counter not resetting could not be determined.Olympus will continue to monitor field performance for this device.
