Event Description
It was reported that the size 3 stem and broach were mismatched.Final stem day up in canal and a new one was needed.

Product complaint # (b)(4).Investigation summary: the device associated with this report was returned for analysis.Examination of the device was not able to confirm the complaint since the involved trial was not returned for examination.A dimensional inspection of the device was performed and met specifications.No defects that could have contributed to a product malfunction were identified.Depuy synthes considers the investigation closed at this time.Should additional information be received, the information will be reviewed and the investigation may be re-opened as necessary.Device history lot: a search of the depuy nonconformance (nc) quality system found no nc¿s associated with this product/lot combination.
