The customer suspected device was returned for investigation.The olympus service center confirmed the reported event during inspection and testing.Upon evaluation of the returned device the following defects were found, pierced / cut or scratch of the bending section, universal cord/ the video cable pierced / cut / or scratched, angle control lever was stuck and could not move, and the guide pin of the skeletons inside the bending section was warped (deformed).The faulty parts were replaced, and the device was returned to the user facility.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
The customer reported to olympus that during reprocessing of the uretero-reno videoscope, water leakage was noticed.Upon inspection and testing of the customer returned device, the scope angle control lever was stuck and could not move.This medical device report (mdr) is being submitted to capture the reportable malfunction found during evaluation.There was no patient involvement, no harm or user injury reported due to the event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 4 years since the subject device was manufactured.Based on the results of the investigation, a definitive root cause could not be determined.However, it is likely due to the angulation wire was stuck in coil pipe by pitch slide of the pipe or the wire corrosion in the pipe.The following information is stated in the instructions for use (ifu): ¿ never perform angulation control forcibly or abruptly.Never forcefully pull, twist, or rotate the angulated bending section.Patient injury, bleeding, and/or perforation may result.It may also become impossible to straighten the bending section during an examination.¿ ¿inspection of the endoscope : inspect the external surface of the entire insertion section, including the bending section and the distal end for dents, bulges, swelling, scratches, peeling of coating, holes, sagging, transformation, bends, adhesion of foreign bodies, missing parts, protruding objects, or other irregularities.¿ ¿do not operate the angulation control lever with excessive force in a narrow space to the opposite direction from the bending direction while the distal end of the endoscope is not moved.The bending section may be damaged.Check the tip position of the endoscope and the shape of the bending section using fluoroscopy, etc.¿ olympus will continue to monitor field performance for this device.
