Event Description
|
An attempt was made to use one telescope guide extension catheter and one launcher guide catheter in a lesion.The launcher device was used with the telescope device and contrast agent was applied without fixating the device.It was reported that the telescope migrated to the coronary artery.It was detailed that the telescope does not move even if it is not fixated normally, but this time the telescope advanced to the coronary artery.It was stated that this event is not a defect but rather a problem with the usage of the device.The device was not held in place when applying the contrast agent.There is no problem with the telescope device.There was no health damage to the patient.There is no complaint against the launcher device.
