Event Description
The olympus representative reported on behalf of the customer during cleaning, the visera cysto-nephro videoscope presented with an air leak on the distal end.No patient injury or procedure impact was reported.During the evaluation of the device, it was noted that there was scraping on the forceps plug cap.This report is to capture the reportable malfunction of the scraping on the forceps plug cap noted at estimation.
 
Manufacturer Narrative
Date of event/event: no additional information was available.The device was evaluated by olympus.The evaluation found the air leak on the distal end was confirmed.The forceps plug cap had been scraped off.The water tightness was not maintained due to holes in the bending section and perforation in the forceps channel.Wrinkles were found in the insertion tube and the bending angle was insufficient due to the elongation of the angle wire.The universal cord was collapsed, the video cable and the flexible tube of the insertion section were crushed, and the video connector case was cracked due to external factors.Scratches were found in multiple locations on the device.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 10 years since the subject device was manufactured.Based on the results of the investigation, a definitive root cause could not be established.The suggested phenomenon was presumed to have been due to stress or handling.Olympus will continue to monitor field performance for this device.
