The subject device was sent at regional repair center olympus france (rrc-ofr) for inspection and repair.The device evaluation still in progress and final evaluation report has not yet been finalized.Supplemental report(s) will be submitted should any relevant new information is available.Investigation is ongoing.This report will be supplemented accordingly following investigation.
 
Event Description
Customer reported with an issue of "air/water flow issues from the air/water flow system".The issue found during reprocessing.There was no patient harm, no user injury reported due to the event.Device return initial evaluation found clogging nozzle, foreign material cannot be removed due to buckling of the nozzle.This reported is being submitted for foreign material found and cannot be removed from the nozzle due to buckling.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the root cause of the foreign material in the nozzle could not be determined, however, based on information provided by the customer, the issue is likely the result of the reprocessing deviating from the instructions for use (ifu).In addition, the following non-reportable malfunctions were found during the device evaluation: - a-rubber glue is worn out and separated - one lg-rod lens unit is cracked - air/water flow issues from the air/water flow system olympus will continue to monitor field performance for this device.
