No additional information was available.The device was evaluated by olympus.The evaluation found that the allegation was confirmed.The scope presented a b31 scope communication error.An additional reportable malfunction was identified.The information identification function was lost due to the breakage of the identification board.Additional issues were identified.The hard tube was discolored, the curved pipe blade was broken, and the bending section adhesive part was missing.The vent metal on the light guide connector was loose scratches were found in multiple locations on the device.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation.
 
Event Description
The olympus representative reported the endoeye flex deflectable videoscope was displaying a b31 scope communication error code.The same scope communication error code was displayed even when the scope was connected to other systems.No patient injury or procedure impact was reported.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the root cause of the events were unable to be identified; however, the events most likely occurred due to a failure of the image sensor unit or a problem with the circuit board inside the video connector.Olympus will continue to monitor field performance for this device.
