Event Description
The customer reported to olympus that there was a water leak, while using the uretero-reno videoscope.The issue was found after the therapeutic procedure (f-tul), there was no delay and the procedure was completed with the same set of equipment.There was no patient harm associated with this event.The device was returned and evaluated, and it was found that the curved rubber adhesive had fallen off and was missing.This mdr is being submitted to capture the reportable malfunction found during the device evaluation.
 
Manufacturer Narrative
The device was returned and evaluated and the customers allegation was confirmed; watertightness was not maintained due to holes in the curved rubber.The curved rubber adhesive had fallen off (was missing), the bending angle was over, a cut on the angle wire was recognized, the forceps could not be inserted smoothly into the forceps channel, scratches were found on the insertion tube and scratches were found on the operation part.Scratches were also found on the following parts: the rotating mechanism, the grip, the universal cord, the video cable, the video connector, the video connector case, the light guide connector, and the up/down angle fixing lever.The investigation is ongoing and a supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, a definitive root cause could not be determined.However, it is likely due to external factors or handling.Olympus will continue to monitor field performance for this device.
