In speaking with olympus technical assistance center (tac), the customer noted that they were using a third party sdi (serial digital interface) to hdmi (high-definition multimedia interface) converter and a samsung 55 inch consumer tv.The customer confirmed that both sdi outputs were outputting an image but could not confirm whether the converter was outputting a signal.At the time of the initial call the issue had not resolved, however upon a follow up call with tac the customer reported that the issue was resolved with no further details.The investigation is ongoing and follow up with the user facility is currently being performed.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
The customer reported to olympus technical assistance center (tac), the evis exera ii video system center was not connecting to the slave monitor.There were no reports of patient or user harm associated with this event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.Three attempts were performed to obtain additional information, but no response was received from the customer.The device was not returned; therefore, the reported phenomenon or condition of the device could not be confirmed.The device history record was unable to be reviewed for this device since the serial number was not provided.However, olympus only releases products to market that meet all manufacturing specifications and final product release criteria.The specific root cause of the reported problem could not be determined at this time because the device was not returned.Olympus will continue to monitor field performance for this device.
