Event Description
The olympus representative reported the endoeye flex deflectable videoscope presented an e216 scope communication error message (no image) when connected to the visera elite ii video system center.The error message did not disappear.When connecting other scopes, the e216 error disappeared, therefore it was determined the scope was defective.No patient injury or procedure impact reported.
 
Manufacturer Narrative
Date of event and event no additional information was available.The device was evaluated, and the reported issue was not confirmed.The bending section cover adhesive was missing.Water tightness was not maintained due to holes in the video cable caused by external factors.Scratches were found in multiple locations.Third party repairs were in progress.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.Correction to h10 regarding third party device service.There was no report of the device being serviced by a third party.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, an, a specific root cause could not be identified.However, it is probable that the loss of image was caused by the failure of the image sensor unit, or the failure of the internal circuit board of video connector or the system caused the event.Olympus will continue to monitor field performance for this device.
