Event Description
The customer reported that the spare lamp on his olympus evis exera iii xenon light source was indicating, even though the primary lamp was functioning properly.The customer also reported that an e207 error code was present.This event took place during preparation for an esophagogastroduodenoscopy procedure.Reportedly, the intended procedure was completed with the subject device.There was no patient harm or consequence reported as a result of this event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the device evaluation and the legal manufacturer's final investigation.The olympus service center confirmed the emergency lamp indicator is blinking and the error 207 is displayed due to a defective spare lamp.In addition, minor scratches were found on the housing and back panel.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 3 years since the subject device was manufactured.Based on the results of the investigation, it was confirmed the event occurred due to a faulty emergency lamp.However, the specific root cause of the faulty emergency lamp could not be determined at this time.Olympus will continue to monitor field performance for this device.
