Event Description
Upon completion of the anterior mitral lesions in areas of dispersion, a steam pop was felt. The patient was monitored on intracardiac echocardiography (ice) and a drop in blood pressure followed by progressive posterior pericardial effusion was noted. The patient's blood pressure dropped but maps remained above 60 while the patient was administered vasopressors. At this point, the overall ablation procedure was deemed complete and terminated. The physician moved forward with pericardial drainage using a long needle, wire, dilator, and catheter to remove excess blood. The patient's blood pressure stabilized and the patient was intermittently administered vasopressors. The patient was cardioverted back to sinus rhythm and the decision was made to intubate. The patient underwent mvr, maze, and atrial appendage litigation. The patient remained hemodynamically stable and had been typed and crossed prior. The patient was transferred to the intensive care unit (icu) to be monitored.Serial echocardiograms revealed that the effusion had resolved. It is believed that the pericardial effusion was caused by the steam pop.
