Event Description
It was reported that the bd q-syte luer access split septum was adhered to the infusion set.The following information was provided by the initial reporter, translated from chinese to english: the head nurse of the emergency department reported that when the infusion set was removed after the infusion was given to the patient, it was found that the infusion connector diaphragm was adhered to the infusion set.

H.6.Investigation summary: our quality engineer inspected the photograph submitted for evaluation.Bd received one photograph which displayed a q-syte device with a luer lock device attached to the female port of the q-syte.The top body column is seen broken, and the septum appears stretched.The reported issue of component damage is confirmed.Although the reported issue was confirmed, the photograph provided for this incident did not present sufficient evidence to identify a definite root cause.Examination of the actual product involved may provide clarification as to the cause for the reported failure.A device history record review showed no non-conformances associated with this issue during the production of this batch.Complaints received for this device and reported condition will continue to be tracked and trended.Information will be captured on trend reports and monitored.Our business team regularly reviews the collected data for identification of emerging trends.
