Event Description
A physician was using amphirion deep for the treatment of an 80mm fibrous lesion with chronic total occlusion in the posterior tibial artery.The device was prepped as per the ifu with no issues identified.A non-mdt inflation device was used and nacl plus contrast medium as inflation fluid.Two balloons were used in the procedure.It was reported that the first balloon had difficulties with inflation, first to nominal after to burst and after 20 bar still no contrast to see.Pressure went back to 12 bar and deflate the balloon was not possible, so they had to puncture through the skin to get the balloon out the patient.Afterwards they used a second balloon same size.No problems with inflation for this balloon but with deflation direct blood was in the catheter and inflation device but the contrast stayed in the balloon.After a while some contrast slowly came out and they could get the balloon out of the patient.No further patient injury reported for this event.
