Event Description
|
On (b)(6) 2022, it was reported by a sales representative via email that an ar-12990 knee scorpion top jaw broke off inside the patient's joint.This was discovered during a meniscal root repair procedure.Surgeon was able to remove all broken fragments.

Complaint confirmed.One unpackaged ar-12990 knee scorpion serial/batch number (b)(6) was received for investigation.Functional testing found that the device does not function as required.Visual evaluation found that the device was broken off at the distal end missing the top jaw.The fragments generated during the event were not returned for analysis.The observed condition is most likely the result of applying excessive force while grasping and manipulating the suture.
