Event Description
The customer reported to olympus that while using the visera elite xenon light source, the lamp exploded inside the device and an e102 error occurred.It was also reported there was plenty of fluff inside the device and the explosion was contained inside the device.The issue occurred during/toward the end of the diagnostic procedure (cystoscopy); the procedure was completed with the same set of equipment and there was no patient harm associated with the event.

The device is not expected to be returned for evaluation.The investigation is ongoing, and a supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 10 years since the subject device was manufactured.Based on the results of the investigation, the phenomenon, error e102, may be attributed to faulty main lamp that has a crack.Since the device was not returned for evaluation, the definitive root cause of the reported issue could not be determined.The following information is stated in the instructions for use (ifu): "2.Troubleshooting for error code message: faulty clv lamp error e102 the phenomenon: check illumination lamp.The error alarms defect temperature switch on the device (temperature increased inside the light source device and the safety mechanism has been operated.)." olympus will continue to monitor field performance for this device.
