The customer reported to olympus, the uretero-reno videoscope experienced a leakage approximately 1 centimeter from the distal end.The event occurred during reprocessing.There was no report of patient harm associated with this event.During incoming inspection, it was determined the bending steel structure broke through bending section cover.This medwatch is being submitted to capture the reportable malfunction found during evaluation.

The subject device was returned to olympus for evaluation.During inspection and testing, the allegation of leakage approximately 1 centimeter from the distal end was not confirmed.In addition to the finds reported in the event, the dial unit was damaged.The bending section cover was leaking.The universal cord was wrinkled.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, a definitive root cause could not be established.The suggested phenomenon was presumed to have been due to excessive force was applied to the passive bending section.Then the cable support got separated and protruded from the a-rubber (distal sheath).The user may be able to reduce / prevent occurrence of the suggested event by handling device in accordance with the following instructions for use (ifu): "·do not strike, hit, or drop the endoscope¿s distal end, insertion tube, bending section, control section, universal cord, video connector, or light guide connector.Also, do not bend, pull, or twist the endoscope¿s distal end, insertion tube, bending section, control section, universal cord, video connector, or light guide connector with excessive force.The endoscope may be damaged and could cause patient injury, burns, bleeding, and/or perforations.It could also cause parts of the endoscope to fall off inside the patient.·never perform angulation control forcibly or abruptly.Never forcefully pull, twist, or rotate the angulated bending section.Patient injury, bleeding, and/or perforation may result.It may also become impossible to straighten the bending section during an examination." olympus will continue to monitor field performance for this device.
