Event Description
It was reported that during surgery, while impacting the tibial implant, a piece of the instrument broke off.Nothing left in the patient.No consequences or impact to the patient.Due diligence is in progress for this complaint; to date no additional information or product has been received.

(b)(4).Report source ¿ foreign ¿ canada.Investigation of this incident is currently ongoing.Once the investigation is completed, a supplemental medwatch 3500a will be submitted.

(b)(4).This follow-up report is being submitted to relay additional and/or corrected information.Visual examination of the returned product confirms the adjustable back foot has fractured off.The instrument exhibits indentations on the top of the main body.There is also evidence of indentations on the back of the instrument around the adjusting area.The instrument has been in the field for approximately 13 years and 3 months.Review of the device history record identified no deviations or anomalies during manufacturing.Device is used for treatment.Medical records were not provided.A definitive root cause cannot be determined.If any further information is found which would change or alter any conclusions or information, a supplemental report will be filed accordingly.Zimmer biomet will continue to monitor for trends.
