A diamondback 360 coronary orbital atherectomy device (oad) was used to treat a heavily calcified 4mm lesion in the distal left main artery.The viperwire advance guide wire was advanced through the lesion without difficulty.Six low-speed treatments and three high-speed treatments were performed.When the physician attempted to retract the oad, they felt resistance.A perforation of the ostial left main artery was observed with immediate and massive hypotension.Epinephrin was administered.Pericardiocentesis was performed to resolve the cardiac tamponade.The patient was unable to be intubated, therefore, tracheostomy was performed.In addition, cardiac massage, cardiopulmonary resuscitation (cpr) and extracorporeal circulation were performed.Despite having two stents implanted, the perforation persisted, and the patient expired.
